Field Insurance

Every team needs to complete an application for field insurance every year. This applies to:

  • existing team playing at the same fields,
  • existing teams moving to new fields,
    and
    of course
    this definitely applies to
  • new teams. 

For 2024, the way injury reports are to be submitted has changed. Injury reports need to be completely 100% online through the USASA website. When the injured person has completed their portion, their form will be opened up for the MSA administrator to review and to approve. There are no longer any paper forms, and there are no longer any PDF forms. All forms have to be done through the USASA website at: https://www.usadultsoccer.com/page/show/949935-insurance

On USASA’s injury reporting page, there are instructions on how to complete their form. So please refer to that page (https://www.usadultsoccer.com/page/show/949935-insurance) for guidance. The ‘reporter’ on the form is generally the injured player. All additional information needed on the form is that of the injured player. Once the claim is submitted via the USASA site, the injury report will automatically be sent to the MSA verification officer (Dyann Pugliese) based on your answer in the Association section. If the injury claim is legit, your member association will approve it. After the claim is approved, you will receive an email from the USASA/MSA/GLWSL insurance carrier (A-G) with your claim number. You will also receive a hard copy in the mail about 1-2 weeks after the claim number is issued. Further instructions for submitting medical bills will be included in the acknowledgment letter from AG.

Despite the field insurance form talking about sending it to MSA, you should not send your field insurance application to MSA because MSA will be confused when they get it from you versus getting it from the league. Field insurance contracts must be received by MSA 48 hours prior to any scheduled match or that match will be canceled. In order to have your insurance contract in at a timely manner, GLWSL requires you submit your paper work to the league ten (10) working days prior to the scheduled match to avoid complications.
